Handover Note / internal memo on handing over internal matters/responsibilities with the departure of employee/staff. 

It includes (i) the current duties of the leaving employee, (ii) regular/re-occurring meetings, reports or procedures that the leaving employee had to attend or prepare, and (iii) approval and reporting requirements of the position. It may also include the status of current projects/reports/issues to follow up.

Staff members who are about to leave their positions, either temporarily or permanently, should send this handover note to assist their successor in carrying out their duties.

The reasons in the memo could include personal matters, holiday or any other accepted reasons for leave / departure.
