MISSING MILEAGE REQUEST

I attach my missing mileage request form for missing mileage from my in this year.

I hope you are able to credit me with the missing miles. [This flight back to the UK was arranged very quickly because I received a phone call to say that my mother was ill and was being taken into hospital. I arranged the flight and flew home very quickly－I remember my travel agent telling me that if I paid a few hundred dollars extra, my flight would be eligible for air miles, so I chose to do this. However, when I got to the check-in desk at the airport, I was very upset and anxious, and I don't recall reminding the check-in clerk that I was a member. Perhaps this is why my account has not been credited with the extra miles?]

As you will see from my receipt, I did mention that I was a member when I picked up my hire car at airport. However, again these miles have not been credited to me.

I hope that you can look into this and that I will be credited with the air miles that are due from both my air travel and my hire car rental.
